Characteristic

Non-selective beta-blocker.

pharmachologic effect

Pharmacological action - antianginal, hypotensive, antiarrhythmic.

Pharmacodynamics

Reduces the automatism of the sinus node, the excitability of the myocardium, the appearance of ectopic foci of excitation; has a membrane-stabilizing effect, reduces heart rate, slows AV conduction, reduces myocardial contractility and myocardial oxygen demand.

Has hypotensive effect, which stabilizes towards the end of the second week of the course appointment. Reduces the activity of the renin-angiotensin system. Increases atherogenic properties of blood.

It increases uterine contractions (spontaneous and caused by means stimulating the myometrium), which helps to reduce bleeding during labor and during the postoperative period.

Increases the tone of the bronchi, in large doses causes a sedative effect.

Reduces intraocular pressure due to oppression of watery moisture production in the eye chamber, does not affect the pupil size and accommodation.

Pharmacokinetics

Quickly absorbed when ingested and relatively quickly eliminated from the body. Stam in the blood plasma is reached after 1-1.5 hours. Bioavailability after oral intake - 30% (the effect of "first passage" through the liver, microsomal oxidation), increases after intake after eating. T1 / 2 - 2-3 hours. Binding to blood plasma proteins - 90-95%. Penetrates through the placental barrier. Excretion by the kidneys - 90%, unchanged - less than 1%.

Interaction

Incompatible with antipsychotic drugs and anxiolytics.

A few days before the anesthesia with chloroform or ether should stop taking the drug.

Against the background of treatment with propranolol should be avoided in / in the introduction of verapamil, diltiazem.

The hypotensive effect of anaprilin is enhanced when combined with hydrochlorothiazide, reserpine, hydralazine and other antihypertensive drugs, as well as ethanol.

Strengthens the action of thyreostatic and uterotonizing drugs; reduces the effect of antihistamines.

With caution appoint together with hypoglycemic agents.

Dosing and Administration

Inside (regardless of the time of eating). With arterial hypertension - 40 mg twice a day; with insufficient severity of the hypotensive effect, the dose is increased to 40 mg 3 times or 80 mg twice a day. The maximum daily dose is 320 mg (in exceptional cases, 640 mg).

With angina, heart rhythm disturbances - in the initial dose of 20 mg 3 times a day; then the dose is gradually increased to 80-120 mg for 2-3 sessions; the maximum daily dose is 240 mg.

For the prevention of migraine, as well as with essential tremor - in an initial dose of 40 mg 2-3 times a day; if necessary, the dose is gradually increased to 160 mg / day
